Bethlehem Woods Nursing and Rehabilitation Earns Deficiency-Free Rating Again


Bethlehem Woods Nursing and Rehabilitation was found to have no deficiencies during an inspection by the Indiana State Department of Health. This is the second year the community has received this rating.

Operated by American Senior Communities, Bethlehem Woods is located at 4430 Elsdale Drive in Fort Wayne. The State Department of Health inspects it and all other communities to assess compliance with federal and state standards of care, such as adequacy of staffing, quality of care and cleanliness of facilities.

A deficiency-free score is the best score the facility can receive and is a distinction achieved by a low percentage of facilities in the state. Receiving two consecutive deficiency-free ratings is outstanding.
